Build your first form

Open Forms → New form. The builder has three parts:

• Palette (left) — drag a field type onto the canvas.
• Canvas (middle) — your form as field workers will see it.
• Properties (right) — appears when you select a field; set its label, whether it's required, and any conditional visibility rules.

Conditional fields

Select a field, then use Show this field only if… to make it appear only when an earlier field has a specific value — useful for follow-up questions ("If yes, describe the issue").

Publishing

Once your form looks right, click Publish. Publishing creates a version — editing a published form doesn't change what's already been captured against the previous version.
